Start with the pet and source document
Collect relevant clinic records, certificates, adoption paperwork and emails, keeping each source distinct. Assign references such as DOC-01 and record the issuer. Match the pet using identifiers actually present; a common name alone may not distinguish animals.
Keep the original paper or file securely. A copied entry is convenient but can lose a signature, identifier or contextual page. Do not alter an original to make an incomplete record look complete. Note missing pages or unclear text openly.
AVMA’s vaccination brochure explains that the veterinarian recommends an individualized plan based on the pet’s needs. This worksheet organizes evidence for that discussion; it does not choose vaccines, intervals, overdue status or acceptance for travel or boarding.
Worked record: clear treatment date, unclear next-date note
Fictional pet Luna has a clinic record with readable administration information and a later attachment with unclear handwriting. A phone reminder was entered without a clinical source. The useful result is a visible uncertainty, not choosing the date that seems convenient.
| Source | Known | Uncertain | Next check |
|---|---|---|---|
| DOC-01 clinic record | Pet identifier and administration date | All pages present? | Request complete copy if needed |
| DOC-02 later attachment | Document issued later | Handwritten next date | Confirm text with issuer |
| REM-01 owner reminder | Manually entered date | No clinical source | Do not label clinician-confirmed |
| Review note | Sources refer to same pet if identifiers match | Next action unconfirmed | Preserve clinic response |
A later certificate issue date does not establish a second vaccination on that date. A scan’s guessed transcription does not resolve handwriting. Leave the field blank or label the uncertainty and request clarification.
When the clinic supplies a correction, preserve its reference and record a dated amendment to your copied entry. Avoid creating another administration event merely because you received a corrected copy of the same record.
Ask the issuing clinic precise questions
Ask whether the document identifies this pet, which date records administration, what the handwriting means and whether there is a recommended next action to record. Ask which original or certificate is needed for the actual purpose; do not assume an app screenshot is accepted.
For travel or boarding, verify current requirements with the relevant authority or provider and your veterinarian. An organized timeline helps find documents but does not establish legal compliance. A vaccine name alone does not establish a certificate’s validity.
Do not put real identifiers, addresses or owner contact information in a public example. Share only what the intended recipient needs through an appropriate method. The CSV does not send messages or transmit records.
VetCue organizes owner-provided information
VetCue’s current listing describes profiles, vaccine/visit/medication/lab timelines, AI scan samples, reminders and emergency summaries. Free users can keep one pet and use three cumulative AI scans. Premium lists more pets/records, reminders, emergency export and scan limits of 40/week on Weekly and 80/week on Yearly.
We publish VetCue. Check extracted fields against originals and read current processing privacy details before optional AI input. A reminder remains an organizational entry. The app is not veterinary diagnosis, treatment, triage or emergency advice.
No automatic certificate validation or import of this CSV is promised. The emergency-information parent covers contacts and immediate reference details. Here the separate task is establishing where vaccination facts came from and what the clinic still needs to confirm.
If the method fits your task, explore VetCue’s listed features on an iPhone. Check current features and plans before downloading.

Use the CSV in Numbers, Excel or a spreadsheet editor. Three fictional evidence rows and a blank row; no medical schedule is prescribed. Replace them with your own observations. The CSV is separate from the app and no email is required.
Questions answered
Can I infer the next dose from the vaccine name?
No schedule is supplied here. Ask the veterinarian for the recommendation appropriate to the pet.
Does my reminder prove a clinic recommendation?
Only an actual clinical source supports that statement; keep owner reminders separate.
Should conflicting originals be overwritten?
Keep them and seek issuer clarification. Record the correction without inventing a new administration event.
