Start with your concern and observed coverage
Write the concern in your own words, such as “unrefreshed on work mornings”, and when you noticed it. Do not assign a disorder from an app score or invent symptoms to fill a checklist. Add the question you want the appointment to answer.
NHLBI suggests discussing concerns with a doctor and considering a diary for a few weeks. Its guidance includes sleep/wake and nap information, alertness, daytime sleepiness and relevant routines. Ask the clinician what format or period is useful; do not postpone needed care merely to finish a template.
State the dates and nights actually recorded. A missing entry is not zero sleep. Retain the diary so your brief summary can be checked rather than becoming the only surviving record.
Worked packet: four recorded nights out of seven
Fictional period: October 5–11, seven nights expected, four recorded and three missing. One manual entry gives 23:00 bed to 07:00 wake, but onset and awake periods were not recorded. That interval is not eight hours of confirmed sleep.
| Field | Example | Do not infer |
|---|---|---|
| Coverage | 4 recorded / 7 expected; 3 missing | Seven complete nights |
| Source | Manual; imported data separate if present | Interchangeable measurements |
| Timing | 23:00 bed, 07:00 wake; onset unknown | Eight confirmed hours asleep |
| Concern | Unrefreshed work mornings | A diagnosis |
| Question | What else should I observe? | A treatment instruction |
For a range or mean, state the denominator and definition. If sources or timing definitions differ, separate them or ask how the clinician wants them handled. Do not silently average a wearable estimate with a manual bed-to-wake interval.
Download the one-page appointment summary template. It contains blank prompts and a fictional coverage example, sends nothing and includes no real patient data.
Keep context and interpretation separate
Use dated observations about naps, routines, drinks, medicines or unusual circumstances relevant to your discussion. Record medicines as actually used and ask the clinician your questions. This page does not recommend starting, stopping or adjusting them.
Two recorded things occurring together do not establish why sleep changed. Label your interpretation separately from the observations, including uncertainty about recall or times. A tidy chart is not a clinical explanation.
If sleepiness affects driving or other safety-sensitive tasks, raise the concern promptly with an appropriate professional and do not drive while sleepy. This preparation aid is not triage or a reason to delay urgent assistance.
Review MornMood exports before sharing
MornMood’s listing describes manual entries, mood/anxiety/habit notes, optional read-only Apple Health sleep import and editable history. Daily logging remains useful without Premium; Premium lists long-range insights, correlations, full history tools, reminders and CSV/report export.
We publish MornMood. It is not a medical device and does not diagnose or treat sleep disorders or anxiety. Check whether a duration comes from manual times or imported data. A manual interval does not establish sleep onset or subtract periods awake.
The summary template is separate from exports and no automatic CSV import is promised. Review records before sharing through the clinic’s preferred secure method and read current privacy details. The parent diary guide covers daily recording; this page prepares a bounded appointment packet.
If the method fits your task, explore MornMood’s listed features on an iPhone. Check current features and plans before downloading.

Use the CSV in Numbers, Excel or a spreadsheet editor. Three fictional summary rows and a blank row; no thresholds, treatment or confirmed sleep duration. Replace them with your own observations. The CSV is separate from the app and no email is required.
Questions answered
Should missing nights count as zero?
No. State the missing count and use defined observations and denominators when summarizing.
Is time in bed the same as time asleep?
Not necessarily; onset delay and awake periods may be included.
Does a correlation explain my symptoms?
It does not establish diagnosis or cause. Bring observations and questions to a qualified professional.
